Achieving better integration for a better health service

With the abolishment of the Medicare Locals & the introduction of the Primary Health Networks, primary health care organisations are facing significant change. Economic pressure, technological innovation and social trends have brought about the transfer of services from hospitals to home and community settings. Demand will increase further as a result of demographic ageing. However, in this time of significant change, the importance of building our communities of practice is even more critical to integrating primary care. This will lead to influencing primary care outcomes and ensuring the focus still remains on the patient.

The conference will look at creating new and innovative service models, improving communication between the federal and state governments, hospitals and primary care organisations and focus on new methods to measure patient outcomes.
